Interventional Procedures Procedures For severe nerve pain in CIDP, your provider may recommend injections or surgical options.e60dc2a1-f33c-4a05-9b50-8e3e8e5976295c6e012f-5beb-4a10-9f06-94bd3db6846d Interventional pain management for CIDP can include:e60dc2a1-f33c-4a05-9b50-8e3e8e5976295c6e012f-5beb-4a10-9f06-94bd3db6846d Nerve Block Your provider injects numbing medicine around painful nerves, which stops you from feeling pain from those nerves.e60dc2a1-f33c-4a05-9b50-8e3e8e597629f711b46c-2770-430e-ba58-161ae36fd5c1 Spinal Cord Stimulation (SCS) Tiny, surgically placed wires send gentle electrical pulses to your spine that block pain signals before you feel them. Peripheral Nerve Stimulation Similar to SCS, peripheral nerve stimulation involves implanting small wires that send electrical signals, but they target a more specific area and localized pain.e60dc2a1-f33c-4a05-9b50-8e3e8e597629e466b7c7-8c88-4153-aec0-9ad033eab18c To help you manage pain, your provider may also adjust CIDP treatments like intravenous immunoglobulin therapy (IVIg) and efgartimod alfa (Vyvgart Hytrulo), which can help decrease inflammation and painful symptoms.e60dc2a1-f33c-4a05-9b50-8e3e8e597629ffe3459d-e499-4f4a-b17e-b15f9e4d372fe60dc2a1-f33c-4a05-9b50-8e3e8e597629978a970b-bc18-4e72-87ab-291a04c580a9
Physical and Complementary Therapies Other Therapies Besides medications and medical procedures, you can lessen your pain with physical therapy and other noninvasive treatments.e60dc2a1-f33c-4a05-9b50-8e3e8e5976295e6a47cf-0c10-40a1-8434-6e648cf9543b “Using physical therapy and encouraging activity is extremely important to assist and improve joint pain or instability around joints due to weakened muscles as well as prevent or improve pain from immobility,” says Hurst. Physical and Occupational Therapy Physical therapy can help you prevent muscle weakness and stiffness, which can cause even more pain.e60dc2a1-f33c-4a05-9b50-8e3e8e5976296e3d9da9-c25f-42e0-9caa-743125b322ec Occupational and physical therapists also offer desensitization therapy, which retrains how your brain manages pain messages.e60dc2a1-f33c-4a05-9b50-8e3e8e5976290f9b59d4-a5c3-464f-bcc6-9e34a66e617f During this treatment, your therapist may place a warm or cold pack on the painful area and gradually adjust the pressure or temperature until your body gets used to the sensation.e60dc2a1-f33c-4a05-9b50-8e3e8e5976290f9b59d4-a5c3-464f-bcc6-9e34a66e617f Transcutaneous Electrical Nerve Stimulation (TENS) Another treatment called transcutaneous electrical nerve stimulation (TENS) sends mild electric currents through electrodes on your skin. TENS can change how your nerves send and receive pain signals. Unlike SCS, TENS doesn’t require a medical procedure, and it can be less expensive and more convenient. TENS works best for small areas of pain.e60dc2a1-f33c-4a05-9b50-8e3e8e5976295e6a47cf-0c10-40a1-8434-6e648cf9543b Acupuncture Acupuncture has been shown to be effective in helping treat neuropathic pain.e60dc2a1-f33c-4a05-9b50-8e3e8e597629dcab9b78-f4bf-452a-9015-b03d04cf22e9 While the research on acupuncture for CIDP specifically is lacking, studies have found acupuncture to be effective and safe in reducing the intensity of neuropathic pain caused by chemotherapy or related to diabetes.e60dc2a1-f33c-4a05-9b50-8e3e8e597629baf8c242-5788-47ef-bd4e-05316f1a7798e60dc2a1-f33c-4a05-9b50-8e3e8e5976299c0c4e9d-78da-4392-a51e-28082d00cd5e Lifestyle Changes You can lessen nerve pain by adding regular exercise to your weekly routine.e60dc2a1-f33c-4a05-9b50-8e3e8e5976295e6a47cf-0c10-40a1-8434-6e648cf9543b Many people with nerve pain find that walking and swimming work well for relief.e60dc2a1-f33c-4a05-9b50-8e3e8e5976296e5e0825-54ec-40a8-bc4d-f98cccb34baf A healthy anti-inflammatory diet — with nutrient-dense foods such as fresh fruits and vegetables, whole grains, nuts, and fish — can be an important part of managing autoimmune disorders like CIDP. This also means avoiding processed foods and foods high in saturated fats and added sugars.e60dc2a1-f33c-4a05-9b50-8e3e8e59762937b7f766-f482-4264-b69f-1cd30361ee91 Psychological Therapy Psychological therapies can also help you manage CIDP pain, alongside other symptoms, says Dr. Lynch. “What’s important to recognize is that pain in CIDP, like with other chronic diseases, rarely exists in isolation — depression and fatigue so often coexist. It’s about treating the whole patient, not just the nerve.” Mental health therapies like cognitive behavioral therapy (CBT) , acceptance and commitment therapy (ACT), and pain reprocessing therapy can change how your brain processes and copes with pain.e60dc2a1-f33c-4a05-9b50-8e3e8e5976295e6a47cf-0c10-40a1-8434-6e648cf9543be60dc2a1-f33c-4a05-9b50-8e3e8e59762970b32b27-4411-4edb-9831-edb59fc6a23be60dc2a1-f33c-4a05-9b50-8e3e8e59762907fcb7b6-3586-4504-9ae9-cc04a4f6649c
Topical and Local Treatments Topical Treatments Your provider may also recommend treatments that go straight onto your skin. Topical treatments don’t work well for large areas, but you can use them for specific areas of neuropathic pain — usually the feet, says Hurst. Topical treatments include:e60dc2a1-f33c-4a05-9b50-8e3e8e597629e115e4a6-a6b1-4c4d-90fa-0766c5e8a42b Lidocaine Patches You place these medicine-infused stickers onto your skin over the area you feel nerve pain. Lidocaine works by blocking pain signals, which makes the area feel numb. Capsaicin This medication comes as a cream or a patch and makes the areas it’s applied to less sensitive to pain. It activates pain fibers (causing a warm or burning feeling) and then reduces the amount of pain chemicals those nerves release. With repeated use, the nerves become less sensitive, so pain decreases.
Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) pain can feel like burning or stabbing sensations in your arms and legs, and CIDP treatment doesn’t always help these symptoms.e60dc2a1-f33c-4a05-9b50-8e3e8e597629975d5be4-ad8d-4321-9598-997b2c8cce44e60dc2a1-f33c-4a05-9b50-8e3e8e5976297d1dbe1c-1c5b-477f-aaf0-0e86295363c2e60dc2a1-f33c-4a05-9b50-8e3e8e59762955dc106d-d213-45e8-ba28-024a00e6fe53 Almost half of everyone with CIDP feels this kind of neuropathic (nerve) pain at some point, and pain management plays an important role in treatment.e60dc2a1-f33c-4a05-9b50-8e3e8e59762945f9091e-5794-4d62-85ab-21f84c66c71ae60dc2a1-f33c-4a05-9b50-8e3e8e5976296a1f3c4a-e757-4198-9f78-ef081842f101 “Pain management for patients with CIDP is very individualized,” says Rebecca Hurst, MD , an associate professor of neurology and the chief of neurology at Tampa General Hospital in Florida. Dr. Hurst tailors pain treatment for each of her patients, depending on their preferences, other medical conditions, and how well they respond to medications. There’s not a lot of research to definitively say what works best when it comes to pain treatment in CIDP.e60dc2a1-f33c-4a05-9b50-8e3e8e59762955dc106d-d213-45e8-ba28-024a00e6fe53 But your healthcare provider can work with you to find pain management options that decrease your discomfort.
How Does CIDP Cause Pain? How CIDP Causes Pain When you have CIDP, your immune system attacks the protective coating (myelin sheath) around your nerves. The inflammation that happens during this process can damage this sheath, leading to weakness, numbness, and pain in affected areas.e60dc2a1-f33c-4a05-9b50-8e3e8e597629b0e7703f-62ea-4ab3-a557-7da645ebade8 “Pain in CIDP is one of the most underappreciated dimensions of this disease — and one of the most impactful on quality of life,” says Karen Lynch, MD , a neurologist at Boston Medical Center in Brighton, Massachussetts, and the global medical director of neurology at Sanofi. “My patients describe it as burning, stabbing, or an unrelenting ache that may not respond well to standard [pain medications].” Some people first notice these symptoms in their fingers and toes before these symptoms travel further up their arms and legs.e60dc2a1-f33c-4a05-9b50-8e3e8e5976294f77d699-7a4d-4328-a4d6-fb0aff6773ab But you can also feel other types of pain with CIDP. For example, nerve damage can cause muscle weakness that leads to reduced activity, altered biomechanics, and muscle strain, and patients may develop secondary muscle pain, cramps, and experience a decline in overall mobility.e60dc2a1-f33c-4a05-9b50-8e3e8e597629f0a4ca3f-5440-455c-926f-f2913389f930

Oral Medications for Pain Oral Pain Medications Research doesn’t say much about treating pain in CIDP. But many providers turn to established treatments for nerve pain caused by other conditions, alongside immune therapy and other treatments for CIDP , like corticosteroids which calm down the immune system and lessen inflammation.e60dc2a1-f33c-4a05-9b50-8e3e8e5976293f1801ef-dfb5-45b4-bda0-f5b0103d665be60dc2a1-f33c-4a05-9b50-8e3e8e597629598e4e6c-60b6-45c5-8499-3b36a4c890ffe60dc2a1-f33c-4a05-9b50-8e3e8e597629a01bc2c8-2bfa-469b-b70d-6d6f79853d1f Oral medications used for nerve pain in CIDP won’t treat the underlying neuropathy but they can help ease symptoms. These include:e60dc2a1-f33c-4a05-9b50-8e3e8e597629a01bc2c8-2bfa-469b-b70d-6d6f79853d1fe60dc2a1-f33c-4a05-9b50-8e3e8e59762985c8fe35-42c4-4a90-b4a7-5b5575cea408e60dc2a1-f33c-4a05-9b50-8e3e8e597629598e4e6c-60b6-45c5-8499-3b36a4c890ff Antidepressants : Tricyclic antidepressants like amitriptyline, nortriptyline, and serotonin norepinephrine reuptake inhibitors (SNRIs) like duloxetine (Cymbalta) and venlafaxine (Effexor) can change how your body sends pain signals from nerves to your brain. Anticonvulsants like pregabalin (Lyrica) and gabapentin (Neurontin) make nerves less sensitive. For nerve pain, Hurst suggests starting with gabapentin or pregabalin and considering tricyclic antidepressants. “We use anticonvulsants and antidepressants to target specific brain messengers that prevent damaged nerves from getting overexcited and sending too many pain signals,” says Hurst. Some providers may offer short-term opioid treatment like tramadol (Ultram) for severe pain. But these medications can cause dependence and come with side effects like drowsiness and constipation. They aren’t usually the first choice for pain management in CIDP.e60dc2a1-f33c-4a05-9b50-8e3e8e597629598e4e6c-60b6-45c5-8499-3b36a4c890ffe60dc2a1-f33c-4a05-9b50-8e3e8e59762985c8fe35-42c4-4a90-b4a7-5b5575cea408
